The Short Answer
If you have diabetes, Medicare Part B covers a dilated diabetic eye exam once every 12 months, performed by an eye doctor who's legally authorized to do the test in your state. This exam looks for diabetic retinopathy and other diabetes-related eye changes — and it's one of the most important things you can do to protect your sight.
What's Covered — and What Isn't
Covered: A yearly medical eye exam for diabetic retinopathy. This is considered medically necessary care because diabetes puts your vision at real risk, so it falls under Medicare Part B (medical), not vision insurance.
Usually not covered by Original Medicare: Routine vision exams just for glasses or contacts, and the glasses themselves (with limited exceptions, such as after cataract surgery). A diabetic eye exam is different — it's a medical exam, not a glasses check.
Costs: With Original Medicare you'll typically pay your Part B deductible and a share of the cost (coinsurance). If you have a Medicare Advantage (Part C) plan, it covers at least what Original Medicare does and often adds extra vision benefits — but the rules and costs vary by plan.
